99 GSX Drive shaft wore hole thru hull

My 99 GSX Limited wore a hole under the drive shaft at the boot. It appears one of the screw clamps somehow spun around on the shaft and literally wore a hole in the hull. An

Any ideas on what might be the cause? I can't see anything wrong with the drive shaft or motor mounts at this point.
