That is a water jacketed pipe so you need to have cooling water going into the outer chamber and is why there is two fittings for the jacket, one in and one out. Typically it would fill from the bottom so all the air is purged out of the high point.
The smaller fitting at the very end at a 45 degree angle is to inject water into the actual exhaust stream to cool the rubber fitting and water box.

So where can i get a couple of the fittings?
